Responsibilities include but not limited to:

  • Participate in daily ward rounds, morning reports, and lectures, to discuss treatments with other healthcare professionals.
  • Participate in the unit dose system, by checking daily doses and accepting/approving them after confirming the appropriateness of the dose and indication, or refusing them with mentioning the reason and providing the recommended intervention.
  • Prepare special pharmaceutical formulas for patients.
  • Evaluate the appropriateness, effectiveness, safety, and compliance of the patient's medications.
  • Write clinical guidelines and policies for drug and therapy within the hospital, regarding medication dilution, extemporaneous preparation, storage, and compatibility.
  • Correlate doctors medication orders and nursing administration of drugs by ascertaining the correct drug name, dose, strength, frequency, duration of therapy, and indication wih regards to the medical condition.
  • Engage in the antimicrobial stewardship program parallel to the hospital infection control committee, which provides complete and important counseling for using the proper antibiotic according to the national guidelines.
  • Provide advice to other healthcare professionals on the effects of medication combinations, interactions, incompatibilities and stability of medicines.
  • Provide advice on doses and suggest the most appropriate dosage form of medication and route of administration.
  • Follow the medication reconciliation process on the patient’s file (admission reports, discharge report, drug history, and medical lab results) in order to prevent any drug interaction or duplication.

Qualifications:

  • 6-year bachelor degree of Pharm D or Master Degree in clinical pharmacy.
  • Valid professional license to practice issued by the ministry of health.
  • Excellent understanding of pharmaceutical therapy and direct patient care.
  • Extensive knowledge in assessing medical cases and creating suitable treatment plans.
  • Solid knowledge of drug preparation, dilution and administration.
  • Proficient knowledge of computers and Microsoft programs.
  • Outstanding communication skills.
  • Strong professional ethics.
  • Time managment and flexibility outside working hours.
